About
Service Coordination, Inc. is a Maryland nonprofit that helps people with intellectual and developmental disabilities, older adults and people with complex medical needs find, plan and coordinate the services they qualify for. It is based in Frederick and has no walk-in office in Garrett County, but its coordinators and planners serve Garrett County families, meeting people at home.
Coordination of Community Services
Through a partnership with the Maryland Developmental Disabilities Administration (DDA), Service Coordination provides Coordination of Community Services (CCS) for people of all ages with intellectual and developmental disabilities and their families. A coordinator helps you:
- understand DDA services and the eligibility process
- build a person-centered plan and adjust it as needs change
- stay healthy, safe and secure at home
- find other community resources while waiting on the DDA Waiting List
Maryland's 211 directory lists Garrett, Allegany, Washington, Frederick, Carroll, Howard and Montgomery counties in its CCS service area, with in-home visits for ongoing support.
Supports Planning for Medicaid Programs
The agency's Supports Planning Services (SPS) help people with complex medical needs work through Medicaid long-term services and supports, including the Community Options Waiver and Community First Choice. Supports Planners help with eligibility and enrollment steps, person-centered planning, coordinating with the providers you choose, ongoing check-ins and moves from a facility back home. The Maryland Department of Health lists Service Coordination among the Supports Planning Agencies serving Garrett County. SPS has its own line, (888) 505-8677.
The agency is accredited by the Standards for Excellence Institute. If you or a family member are starting with DDA services, the CCS line below is the quickest way to reach a coordinator.
